Ohio Home Care Waiver Program

The Ohio Home Care Waiver enables people the ability to receive care in their home and community instead of residing in a nursing home, rehabilitation facility or hospital. Enrollment on the Ohio Home Care Waiver Program is available for Medicaid eligible individuals under the age of 60, who meet an Intermediate or Skilled Level of Care, and other program eligibility requirements. On behalf of the Ohio Department of Medicaid, 丝瓜短视频, in partnership with the local Area Agency on Aging, provide eligibility assessments, community resource referrals, and case management duties to individuals who experience chronic, significant physical disabilities and/or unstable medical conditions. Our Case Managers will work with the person and their chosen team to provide a Person Centered Service Plan. Individuals enrolled in the Ohio Home Care Waiver Program may receive the following services:
